A Practical Referral Pathway for Your Participants

Support coordinators need responsive providers who communicate clearly and keep therapy aligned with the participant’s plan and goals. CEO Therapy provides mobile physiotherapy in the participant’s home or community environment, helping assessment and treatment reflect real functional needs.

With participant consent, we can communicate with support coordinators, plan managers, families and treating professionals. Progress reports and recommendations can also be prepared when required.

When to Refer for NDIS Physiotherapy

A physiotherapy referral may be helpful when a participant is experiencing reduced mobility, falls or near falls, difficulty with transfers, weakness, reduced endurance, pain affecting function, changes in walking or reduced participation in daily activities.

Physiotherapy may also support participants who need an individual exercise program, review of physical capacity, gait or balance rehabilitation, carer education, or recommendations relating to safe movement and mobility.

How to Get Started

01 — Send a Referral

Call 0412 814 247 or submit the enquiry form with the participant’s contact details, suburb, funding type and primary physiotherapy needs.

02 — Confirm Details

We confirm consent, service availability, funding arrangements, relevant goals and the information required before the first appointment.

03 — Assessment & Plan

An initial assessment is completed and a practical treatment plan is developed. Communication and reporting are arranged according to the participant’s needs.

Funding and Service Arrangements

CEO Therapy works with self-managed and plan-managed NDIS participants. For NDIA-managed plans, contact us before referring so we can confirm the relevant provider and funding arrangements.

Please include the participant’s preferred contact, suburb, plan-management type, relevant goals and any important mobility or safety information. Additional clinical documents can be discussed after consent is confirmed.
