Self-managed and plan-managed NDIS participants can contact us directly to discuss their goals and arrange an initial physiotherapy assessment.
Simple referral process: Call 0412 814 247 or submit the enquiry form. We will discuss your goals, preferred appointment time, service location and funding arrangements.
Collaborative care: With your consent, we can communicate with your family, support coordinator, plan manager and treating team. Physiotherapy reports and recommendations can also be prepared when required.
